Position Summary

The Transfer & Coordination Specialist oversees the internal transfer process to ensure clients are transitioned between clinicians in a timely, clinically appropriate, and minimally disruptive manner. This role also supports intake list management and client assignment workflows to maintain continuity in client access to care. By monitoring transfer requests and collaborating with clinicians and supervisors, the specialist helps improve client-clinician matching, reduce delays in care, and maintain the integrity of the client assignment process. This is a supplemental internal role open to current Pillars clinicians, offering a 2-client hour reduction in exchange for 4 administrative hours of weekly responsibilities.

Responsibilities

Transfer Coordination

  • Serve as the primary point of coordination for client transfer requests
  • Review and evaluate transfer requests in collaboration with supervisors and leadership
  • Facilitate timely reassignment of clients to appropriate clinicians
  • Communicate transfer outcomes with clinicians and ensure appropriate documentation
  • Monitor transfer timelines to prevent disruptions in client care

Transfer Process Oversight

  • Maintain a centralized system for tracking transfer requests and outcomes
  • Identify patterns or trends contributing to transfer requests (e.g., scheduling conflicts, specialization mismatch)
  • Provide feedback to intake leadership regarding recurring transfer themes that may inform pairing practices or hiring needs
  • Support development and refinement of transfer policies and procedures

Client Assignment & List Support

  • Assist with client list management and assignment as needed
  • Support movement of clients through intake statuses when the Assignment & List Lead is unavailable
  • Assist in resolving pairing conflicts or complex assignment scenarios
  • Support reassignments when clinicians become unavailable or capacity changes

Operational Collaboration

  • Collaborate with the Assignment & List Lead to maintain efficient client list flow
  • Communicate with supervisors when transfer or reassignment concerns arise

Expectations

  • Work one evening shift (4pm–7pm) each week to complete a minimum of 2 intakes
  • Maintain a 90% or better on intake audits
Qualifications
  • Current Pillars of Wellness approved provider
  • Fully licensed or associate-level mental health professional (LCSW, LSW, LMHC, LMHC-A, LMFT, LMFT-A, LCAC, LCAC-A, LPCC, Clinical Psychology PsyD/PhD, HSPP, or equivalent)
  • National Provider Identifier (NPI) number
  • CAQH profile on file
  • Individual professional malpractice liability insurance policy ($1M/$3M)
  • Strong organizational, communication, and problem-solving skills
  • Ability to manage competing priorities and work collaboratively across teams
